Description

Per DAL 19-08, released April 9, 2019, enabler bars, hospital beds, and ½ side rails have been removed from the equivalency list rendering them full waivers requiring approval by DOH.  Throughout the latter part of 2019 and the subsequent pandemic, in discussion with ESAAL and other associations, DOH representatives agreed that Operators should prepare the waivers but hold them on-site.  The DOH has recently stated that Operators should now be submitting them to their Regional Office.

In this session the presenter will share guidance on preparing and submitting these waivers, what supporting documentation is required, and the necessary elements to satisfy DOH expectations.  She will also provide samples of helpful tools, such as staff training, installation and safety check documentation, that can be adapted to your organization.

She will address the following topics:

  • What equipment is subject to the waiver
  • Conditions for use of the equipment
  • Requirements of documentation from the Medical Provider
  • Completion of the Waiver Request-Equivalency Notification (DOH-4235)
  • Supporting documentation
  • Recent requests by DOH for edits and revisions
