Wandering and Elopement Documentation Workshop

Description

Wandering and elopement can be potential challenges in a variety of settings and are not limited to only dementia care residences.  This session will focus on case management, intervention and mitigation efforts, and documentation for residents who perpetually wander or have a history of or risk for elopement.  The session will be conducted in a highly interactive workshop format using real examples of Case Management notes, Incident Reports, Care Plans, and other required documentation.

Goals for the Session

Identify:

• Strengths and weakness
• Gaps and/or unanswered questions
• Care Plan accuracy and timeliness
• Effective incident management and reporting
