The NYS Dept. of Health releases the 2017 Coastal Storm and Flood Planning Activities 

VIEW the Facility Profile Application Users Guide V 2.0

The New York State Department of Health (NYSDOH) – Facility Profile Application (PA) is a planning tool that provides health care facilities (HCF) (adult care facilities, hospitals, nursing homes) with an easy process to assess and maintain information on key preparedness capabilities and capacities. This includes detailed information about the facility’s patient/resident send – receive arrangements with other HCFs as part of their evacuation planning.

The PA Users Guide is a component of NYSDOH HCF evacuation guidance, including the Facility Profile Training Participant Guide, (Commerce Training Institute, May 2017) and the Healthcare Facility Evacuation Center (HEC) Facility Guidance Document. Both are accessible on the HELP screen of the PA, on the Health Commerce System (HCS)

HCFs should refer to the PA Users Guide and the PA Training Participant Guide (May 2017) as resources when navigating and logging data into the PA.

The PA Users Guide compliments, but does not in any way replace an individual HCF’s evacuation plans, its coordination with the HCF’s respective jurisdiction plans and procedures, or discussions between HCFs as part of their send – receive arrangement planning.
